QUALIFIED THIRD PARTY CERTIFICATE OF ENDORSEMENT TO THE ENERGY REGULATORY COMMISSION (QTP-COE TO ERC)

1. Letter of Request addressed to the EPIMB Director
2. Company Profile
3. Project Background/ Description to include at least the following:
   • Name of the generating facility
   • Nameplate Capacity (including photographs of nameplate attached to the machine)
   • Exact Location
   • Target Commercial Operation; and
   • Jobs Generated (During Construction and During Operation)
4. Copy of Resolution of Award of the QTP BAC duly signed by the QTP BAC Chairperson
5. SEC Registration
6. Articles of Incorporation
7. By-Laws of the Company
8. Copy of QTP Service Contract

Additional for ERC COC Renewal:

1. Copy of the ERC Certificate of Compliance (COC)
2. Certification of new rated capacity (if applicable)

Additional for Renewable Energy Power Projects:

1. Copy of the DOE RE Service Contract or RE Certificate of Registration (Whichever is applicabe)
